Issue 013 documented the inverted evidence burden under DSHEA and the clinical significance of drug-supplement interactions. Issue 014 addresses what the individual patient can do within the current system to reduce their personal exposure to undocumented interaction risk.

The Concomitant Medications Log: Clinical Research Standard
Concomitant medications (conmeds) are all medications, supplements, vitamins, herbal products, and other substances a trial participant takes during a study. Documenting them is a regulatory requirement under FDA's Good Clinical Practice guidance (ICH E6) and is specified in most study protocols as a standard data collection point.
The purpose is multidimensional. At the individual patient level: identifying potential interactions with the investigational product. At the trial level: characterizing the background medication use of the enrolled population, which affects how the safety and efficacy results generalize to the broader patient population. At the regulatory level: providing FDA with a complete picture of the conditions under which the trial was conducted.
Conmed data directly affects how adverse events are interpreted. An unexpected lab value in a trial participant who is also taking a hepatotoxic supplement may be attributable to the supplement, the study drug, or the interaction between them. Without the conmed log, the interpretation is guesswork.

The Disclosure Gap: Why Patients Do Not Disclose
The literature on supplement disclosure in routine care identifies several consistent barriers. Understanding the mechanism is relevant to addressing it.
Provider behavior: physicians don’t consistently ask about supplement use during intake. When time is limited, the structured medication reconciliation question often covers only prescription and OTC medications.
Patient framing: supplements aren’t perceived as medications by a significant portion of the population who uses them. If a patient doesn’t categorize a supplement as a medication, it may not occur to them that it is relevant to report.
Fear of disapproval: some patients anticipate that disclosure will result in a recommendation to stop the supplement, and do not raise the issue to avoid that conversation.
Cognitive load: patients taking multiple supplements may not have a consolidated record of all products, doses, and frequencies at the time of an appointment. Recall under time pressure is systematically incomplete.
Specific Interaction Mechanisms: The Clinical Detail
St. John's Wort (Hypericum perforatum): induction of CYP3A4 and P-glycoprotein reduces plasma concentrations of drugs that are CYP3A4 substrates, including immunosuppressants (cyclosporine, tacrolimus), antiretrovirals (PIs and NNRTIs), benzodiazepines, calcium channel blockers, oral contraceptives, and warfarin. The interaction is clinically significant and has resulted in documented treatment failures, including organ rejection in transplant patients receiving cyclosporine. Multiple regulatory agencies have issued formal warnings.
Grapefruit and grapefruit juice: inhibits CYP3A4 in the intestinal wall, increasing bioavailability of CYP3A4 substrates and increasing plasma drug concentrations. Relevant for statins (particularly simvastatin and atorvastatin), calcium channel blockers, certain immunosuppressants, and some psychiatric medications. The effect can persist for 24 hours after consumption.
Red yeast rice: contains variable concentrations of monacolins, including monacolin K, which is pharmacologically identical to lovastatin. Patients taking red yeast rice alongside a prescribed statin may receive unpredictable combined statin exposure. Product-to-product variability in monacolin content means the interaction is not predictable without product-specific testing.
Berberine: inhibits CYP2D6 and CYP3A4, with potential effects on drugs metabolized by these enzymes. Additive glucose-lowering effects with metformin and sulfonylureas. Has been studied for antiplatelet activity, which is relevant for patients on anticoagulant therapy.
Fish oil (omega-3 fatty acids at high doses): antiplatelet activity is clinically relevant in patients on anticoagulant or antiplatelet therapy. FDA has approved prescription-strength omega-3 products (e.g., icosapent ethyl) for specific cardiovascular indications, which reflects the dose-dependent pharmacological activity of compounds that are simultaneously sold OTC as supplements.

The Adverse Event Log: Patient Self-Advocacy Tool
Clinical trial adverse event documentation requires: event description in plain language, onset date, severity assessment (graded 1-4 or by plain-language equivalent), duration, resolution date, clinical course, and causality assessment relative to the study drug.
The symptom log tool adapts this structure for patient self-documentation between appointments. The primary benefit is recall: patient-reported outcomes are systematically influenced by recall bias, particularly for events that resolved before the appointment date. A log maintained in real time provides a more accurate record than retrospective reconstruction during a 15-minute visit.
Secondary benefit: pattern identification. A patient who notices that a symptom consistently follows a specific meal, medication dose, or supplement addition has data that may be clinically useful. The log is the mechanism that converts that pattern from an impression to a record.

If you work in clinical research or regulatory affairs: the disconnect between conmed documentation standards in trials and documentation practices in routine care is a known and documented gap. Patients who enroll in trials often comment that the intake process gives them more complete information about what they are taking than they have ever had in routine care. The conmed log is a tool that already exists; the barrier to patient use is awareness and access, not complexity.
If yo'u’re a patient or patient advocate: you can request medication reconciliation at every appointment. You can ask your pharmacist to review your full supplement list during a medication review, many pharmacies offer this service. The conmed log is a structured version of what pharmacists and prescribers are trained to collect but do not always have time to gather from scratch. Bringing it completed saves time and improves accuracy. And it is worth handing to the person in your family who cannot fully name what they take: the parent at a cardiology visit, the relative on five prescriptions and a shelf of supplements. The tool does the most good when it leaves your own medicine cabinet.

Q10: If I am wrong about this, what would that mean? Applied to the non-disclosure decision: if a supplement you are taking is interacting with a medication your doctor prescribed, and your doctor does not know about the supplement, what is the mechanism by which anyone identifies the interaction? In a trial, the mechanism is the conmed log and the adverse event reporting system. In routine care, the mechanism is you.
The three moves stack the same way they do in Plain Talk. OPEN lowers the guard (Cora and Nani). PAUSE puts the clinical-research standard on the table (Rooty). The Skeptic's Toolkit tests the decision (Sam and Debby). The Toolkit is the operating system. The conmed log and the symptom log are the instruments. Arc Close: Issues 012-014
The wellness sub-arc ran 3 issues. Issue 012 named what the pharmaceutical industry gets wrong, specifically. Issue 013 asked whether the same standard is applied to the industry that operates without clinical trials. Issue 014 answered the question of what a patient does with all of this in their own life.
The Debby arc ran from Issue 001 to Issue 011 and closed with a commitment. Issues 012-014 are the first test of that commitment in practice. She held the pharma criticism and the pipeline argument at the same time. She brought the supplement bag to the pharmacist. She called her doctor. She downloaded the form.
That is the Skeptic's posture: not certainty, but a process. Not an answer, but a set of questions you are willing to keep asking.
